Minot Public Schools does not send transcripts via electronic methods such as email or fax. Please mail your request using the form below, with the following information.
There is a required $2.00 prepayment per transcript. The only acceptable forms of payment are check, cash or money order.

Please include the following information with your transcript request:
Print off and complete Transcript Request Form (PDF)
Year of graduation or last year attended
Name on records/diploma (please note if there is a current name change)
Indicate where records are to be mailed. (please be as clear as possible)
Your records will be processed and sent the same day that your prepayment and request are received.
